What happens to the information I share on the support request form? This information is primarily just to help us connect and work with you to figure out if Anemoni is what you want/need. However, some questions in this form are based on information we will need to give to our primary funder (the Massachusetts Department of Mental Health) on an enrollment form if you do end up staying at Anemoni House or being supported through our mobile respite. Any question asking for information that would be put on an enrollment form will be marked with **.
You can answer these questions exactly as you want to answer them. (For example, you are not limited to any particular choices for gender, etc.) If you are not comfortable answering these required questions, you can pout N/A or similar.
If you do not end up enrolling, that information will not be shared. We do not check IDs, so you can use a chosen or nick name, etc.
Why do you allow people to reserve a stay at Anemoni House if they’re having Gender Affirming Surgery (GAS)? We understand that – for many people – lack of post-surgery support is a major barrier to being able to have surgery at all. Facilities often won’t let someone schedule a surgery at all without an aftercare plan and somewhere to stay. We also understand that while this can be a joyful time for many people, the healing process can also bring about complicated feelings.
What happens if an Anemoni team member can’t reach me after I fill out a support request form? We will try to reach you at least three times over 24-hours. If we can’t reach you, we will assume you changed your mind. If you do still want Anemoni support, you will need to fill out a new form or call us at Anemoni House to begin moving things forward again.
